3. Section #2.  The first question asks the survey taker what he or she thinks of the subject.  For an example done last summer,  see the survey at  www.ams3.borderlesseducation.org to understand this.

Provide AMS with an outline of what you wish to learn from the first question.

4. Section #3.  The second question  asks the survey taker about herself / himself, but without asking for private information.  See  www.gis3.borderlesseducation.org to understand this. 

 

Provide GIS with an outline of what you wish to learn from the second question.

 

 

5. AMS e-mails you the written survey for your approval.  If you approve, AMS uploads the survey to Google Forms and the survey begins.  Once a survey begins, it cannot be stopped or changed.

6. AMS reserves the right not to survey if AMS believes that a legal violation, ethical issue or controversy  might arise.
